The lines are getting cut off because they are currently tagged as
verbatim areas (in the SGML and in the built HTML), and extend beyond
the margin of the PDF file. The lines in question are over 100 chars
in length, as exhibited here:

   http://www.linuxdoc.org/HOWTO/LDAP-HOWTO-3.html#ss3.2

and in the SGML:

   <tscreen><verb>
   This directive specifies the default access to grant requesters when no
access directives have been
   ...

If those become non-verbatim areas, the problem should be resolved.
Perhaps a "description" list (in linuxdoc) might work to give you
the proper results:

  <descrip>
    <tag/defaultaccess/
      This directive specifies the default access...
