Please, do change this line to:

<!doctype article public "-//OASIS//DTD DocBook V3.1//EN">
                          ^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^

The above line was telling your system to use DocBook V3.0.

I'm using the following strutucture to my e-mail address:

   <affiliation>
      <orgname>
          <ulink url="http://www.LinuxDoc.org";>Linux Documentation Project</ulink>
      </orgname>
      <address><email>amir@infoteen.com</email></address>
   </affilitation>

And then, after all that, don't forget the

</author>

I suggest using the above markup...
